foreskin like a tight rubber band

i have diebetis 1 . a recovering addict. i have my fore skin around my penis that is so tight that it hurts when peeing. then in a erection there is so much pain . its like a rubber band around it . then it pops its head out from for skin. It is very hard for it to come out.
