Nightime Incontinence-Please Help

I am a 39 year old male. I had an injury to my back in Nov of 2005, and had to get emergency surgery as I had a severe problem with bladder control at night. I had disc disectomy preformed on L1,L2,L3L4 and received little relief from the back pain, but no more problems with the urinary incontinence.

For about the past 8 months or so, the incontinence has returned- I can go 3 weeks and not have a single episode and I can have 3 weeks of nightly incontinence. About a week ago I was lying on the floor tightening something on a sink, and for the first time, I felt the urine streaming out of me. I have an EMG and was told I have some permanent nerve damage, but I can not get a straight answer or solution about my bladder problem. Can someone please help me.
