Low Free Testosterone or something else??

I have a situation I would like to get some general advice on. I am 33 years old, athletic and optimistic about life but I have always had erectile problems. From the time I was very young I can only remember possible one or two morning erections and no wet dreams. The only way I can get an erection is through direct stimulation.

I went to two Urologists when I was younger to discuss the problem. The first time I believe I was 19 and still a virgin, for obvious reasons. He did a total testosterone test only and I remember the reading being 843. He prescribed me a Yohimbine medication but it had very little effect. I also had a low libido this whole time but I did date women and became close to a couple that I really had a strong connection with.

After this I decided I was just going to remain celibate and I joined the military. I went to Germany and spent a few years there and then got out and came home. Upon returning I met my wife while I was in college. We were married when I was 25. A short time before then I went to a 2nd Urologist who told me “its just performance anxiety” and prescribed Viagra. It works but still requires some manual stimulation.

Fast forward eight years and three kids later. Still in the same boat. Low libido, somewhat weak erections through manual stimulation and no morning/spontaneous erections. Recently I have been hearing about “free” testosterone levels from some bodybuilding friends of mine, which caught my attention. I have never really discussed my matter with them but took notice of what they said. I remember them talking about when they are coming off of anabolic steroids how they can have extremely high testosterone levels but low free testosterone levels and it would cause them to not have morning/spontaneous erections and a low libido.

I immediately decided to get this checked. The results I received are as follows. I guess my question is who do I take this information to? I live in a small town where the Urologists still think it terms of total testosterone only, didn’t even bother to check other hormones. Are there natural herbal alternatives that REALLY work? Thank you for your assistance. I have considered an Endocrinologist but was looking for some advice first.
